Personal Information for External Instructors
This page lists the information for an external instructor registered in Professional Development. This page stores the contact information your organization will use for the external instructor. To access this page, the participant will click the Personal Information link on the Instructor menu.
To edit the fields on this page, the instructor can click Edit at the bottom of the page. The participant can add or change any information and click Save to save the changes or Cancel to clear any changes made since the last time the page was saved. The participant can click Back to return to the previous page.
Note
This page is only available for external instructors who have registered with your organization. The link will not display for internal instructors using Professional Development.

SSN | Nine-digit Social Security number of the instructor. The system provides the dashes. Character/11, including dashes |
Address Line 1 | First line for the instructor’s address. Character/30 |
Address Line 2 | Second line for the instructor’s address. Character/30 |
City | City for the instructor’s address. Character/20 |
State | State abbreviation for the instructor’s address. Select the state from the dropdown list. |
Zip | Five-digit zip code plus four-digit extension for the instructor’s address. Character/10, including dash |
Home Phone | Ten-digit number plus four-digit extension for the instructor’s home phone number. |
Work Phone | Ten-digit number plus four-digit extension for the instructor’s work phone number. |
Cell Phone | Ten-digit number plus four-digit extension for the instructor’s cell phone number. |
Other Phone | Ten-digit number plus four-digit extension for another phone number for the instructor. |
Email Address | Email address for the instructor. Character/50 |
Alternate Email Address | An alternate email address for the instructor. Character/50 |
School District Location | School district or location for the instructor. Character/50 |
