Print 1095s

Use the Print 1095s page to view and print copies of the 1095 forms available in the Human Resources database.

For 1095 forms and instructions, visit the following IRS pages:

• 1095-C

• 1095-B 

Print 1095 Forms

1. Choose the Print 1095s page.

2. Select the desired 1095 form in the Available years:

   • 1095-C

   • 1095-B

3. Save the form to your computer.

4. Print the form for your records.
